                         Please return the completed Registration Forms back to the parish office at the

                Beaumont Site with the Birth Certificate plus any other documents that we might need 

                such as  Baptism and First Communion Certificates.

                Favor de completar las Formas de Registro y devolverlas a la Oficina Parroquial en el

                Sitio de Beaumont con el Acta de Nacimiento mas qualquier otro documento que

                podamos necesitar  como el Certifico de Bautismo y Primera Comunion.
